Job Description

Position Summary

The Lifestyle Coach drives lifestyle care plans for management, reduction or prevention of disease and risk factors to support the organization and individuals to achieve health improvement goals and overall well-being.

The Coach assesses health risks, navigates to resources and provides case management and behavior modification education, support, and accountability to assigned population with emphasis on lifestyle, behavior change and self-management.

The Lifestyle Coach covers a geographic territory to deliver services to assigned populations and locations and is accountable for a designated patient caseload to facilitate a lifestyle care plan with the underlying objective of enhancing care, optimizing behavioral and clinical outcomes and patient satisfaction while managing the cost of care.

The Coach provides leadership in wellness and population based health improvements such as educational classes and programs for cohorts of patients, environment and culture changes to promote well-being. The coach also helps drive change through performance improvement.

Minimum Requirements

  • Bachelor's Degree or higher from an accredited college or university in a health-related field.
  • Minimum 3 years of relevant coaching or clinical experience.
  • Current NC unrestricted license in one of the following: RDN, LDN, LCSW, LMSW, LPC OR National Board-Certified Health and Wellness Coach, NBC-HWC OR ACSM or ACLM Certified Exercise Physiologist.
  • 2 Advanced Certifications or NBC-HWC required.
  • American Heart Association - Basic Life Support
  • Demonstrates high-level skill in health assessment, care planning, motivational interviewing, and behavior modification. Independently manages assigned caseloads using critical thinking and evidence-based strategies. Collaborates effectively across the care continuum to optimize outcomes and patient engagement.
